To make your first visit a breeze, we have some suggestions and need your help with a few things:

You may fax these forms to our office or bring them to your scheduled appointment to hand in at check-in.

It is extremely important to fill your forms out as accurately and as extensively as possible. Please let Dr. Bennett know of any outside treatment you are receiving such chiropractic, physical therapy, massage therapy, or exercise training, as well as any medical procedures other doctors are performing on you. It is also essential to list all of the medications, vitamins and supplements you are taking and the dosage. We also ask that you bring a food log of the foods and beverages you consumed in the last week. Many people are unaware that their food intake, medications, vitamins, and supplements interact with each other. The more information you bring with you, the better the acupuncturist will be able to tailor a plan to your needs, and the quicker you will notice a change in your health.

Plus bring these items:

  • Driver’s License or Identification Card
  • Insurance Card
  • All lab work, X-rays, MRIs, and CT scans done in the last two years
  • A detailed list of all medications, vitamins and supplements you are taking
  • A weekly food log, simply write down what you ate at each meal for how many days you could recall.
  • An open mind to taking better control of your health.  I am here for you, Dr. Bennett!

What will we accomplish?

Your first visit is the most critical visit of your acupuncture treatments at Bennett Acupuncture.  During this visit you will learn the following SEVEN things:

  1. What your problem is
  2. What caused the problem
  3. What your acupuncturist Dr. Bennett can do for you
  4. How many visits it will take to address your problem
  5. How much money it will cost
  6. If Dr. Bennett  is the right person to help you with your condition
  7. If Dr. Bennett  is not the right person to help you, who she will recommend: Chiropractor, MD, etc.

Make sure to eat something at least two hours prior your scheduled appointment. This doesn’t have to be a large meal, however you want to make sure there is something in your stomach. During acupuncture, your body tends to move things around you may begin to feel hungry if there isn’t already something digesting in your stomach. This can lead to feelings of light headed, dizziness, and a grumbling stomach.

Insurance: We take most insurances, and most insurances cover acupuncture therapy.  We will need your driver’s license to verify your eligibility and benefits through your insurance company.  Knowing your insurance coverage and benefits ensures that you can maximize all of the health benefits you are paying for!  if you have not already given us your insurance information, go ahead and give us a call at 714 – 962 – 5031.
